Join us and become a member!

We welcome you to join Putnam Clubhouse! All members must be Contra Costa County residents who are 18 years or older. To begin the application process, you can call the Clubhouse for assistance at (925) 691-4276 or complete the following steps:

  1. Download and fill out a New Member Enrollment .
  2. Download and fill out an Authorization_Release of Information Form.
  3. Download and have your service provider (psychiatrist, case manager, counselor or therapist) complete the Provider Verification form for Member Eligibility . Your provider must fax this form directly to the Clubhouse at (925) 691-5643 or mail it to Putnam Clubhouse, 3024 Willow Pass Road, Suite 230, Concord, CA 94519.
  4. If you haven't already visited the Clubhouse, call (925) 691-4276 to set up a time to have a tour. You can bring your application and authorization forms with you then. If you have already visited the Clubhouse, please mail your completed forms to Putnam Clubhouse, 3024 Willow Pass Road, Suite 230, Concord, CA 94519.
  5. Once all of your forms have been received, including the Provider Verification, you will be contacted to attend an orientation meeting (providing you have met the eligibility requirements).
  6. After the orientation meeting, you can start attending all Clubhouse activities!
