Part of the daily work of Putnam Clubhouse involves keeping track of members. When a member has not recently attended the Clubhouse, a “reachout” telephone call or visit is made to let the member know that he or she is missed. When a member isn’t feeling well, they will be called and often visited at home or the hospital. The Clubhouse also provides home visits to potential members through the following In-Home Outreach Program.

In-Home Outreach Program

Mental health consumers sometimes feel uncomfortable attending the Clubhouse for the first time, especially if they don’t know anyone there. During an in-home visit, Clubhouse members (current program participants) and staff will pay a fun, social call to the consumer’s home. The person visited is encouraged to attend onsite Clubhouse activities in the future. In some cases, the caregiver will be able to leave the home during the visit, allowing them a break from caregiver duties. Consumers who are already Clubhouse members may also be visited when they are unable to attend onsite programming.   • WHO IS ELIGIBLE FOR A VISIT? The person to be visited must be at least 18 years of age, reside in Contra Costa County, and have a diagnosed mental illness of schizophrenia, schizoaffective disorder, bipolar disorder, or major depressive disorder.
  • WHO CAN REQUEST A VISIT? A caregiver, a family member, or the consumer may request an in-home visit. Regardless of who requests the visit, the person visited must provide his or her consent at the time of the visit.
  • HOW LONG WILL A VISIT LAST AND WHAT HAPPENS DURING THE VISIT? The visit can last up to three hours and will include activities such as watching a movie, playing games, sharing a meal, talking, or listening to music.
  • IS THERE A LIMIT TO THE NUMBER OF VISITS? There is a maximum of three visits per person during a calendar year.
  • HOW DO I REQUEST A VISIT? Download and complete the intake form including all requested information. The completed form can be mailed to Putnam Clubhouse at the above address or faxed to (925) 691-5643 to the attention of Tamara Hunter. An intake form can also be completed by phone by calling Tamara Hunter at (925) 691-4276. After the request form is processed, you will receive a call to finalize arrangements for the visit. While every effort will be made to honor each visit request, all requests require the approval of the Clubhouse Program Director.
